This section must be completed by the competitor. When the competitor is under 18 this section must be completed by the child’s parent or legal guardian.
By ticking a box and entering any UKWA event you are confirming that in your opinion or in the opinion of your parent or legal guardian that you are fit and able to compete. However you may have a medical condition which you might wish to bring to the attention of The Race Committee. Should you wish please complete the medical form and send to admin@ukwindsurfing.com. The information will be shared with the event administrator and the safety team. The information will be treated in the strictest of confidence. All information will be destroyed at the end of the season. Should the information no longer be relevant or should you wish for any other reason you may ask us to remove any stored information at any time by e-mailing admin@ukwindsurfing.com.
Adult/competitor declaration
“I agree to be bound by the rules as defined in the Racing Rules of Sailing, and all other rules that govern this event. I agree to compete in accordance with the RYA Racing Charter. I confirm that I have read the Notice of Race and accept its provisions, in particular the Risk Statement therein. I confirm that throughout the event my boat will have current and valid third party insurance of at least £5m per incident or equivalent. I acknowledge and agree that I am responsible under Rule 3.6 for ensuring that any support persons connected to me during this event will comply with the rules.”
Parent declaration
“Under law, this competitor is my dependent. As a parent and guardian, I acknowledge and agree that it is my responsibility to ensure compliance by my dependent with all rules that govern this .event including the RYA Racing Charter. I confirm that I have read the Notice of Race and accept its provisions, in particular the Risk Statement therein. I confirm that throughout the event the boat sailed by my dependent will have current and valid third party insurance of at least £5m per incident or equivalent. I confirm that my dependent is competent to take part. I will be responsible for my dependent throughout the event, and during the time he/she is afloat I will be available at the event venue, or I will inform the Race Office in writing of the person who is acting in loco parentis. I further acknowledge that I will be treated as a support person under RRS Rule 3.2 and that I must myself comply with all rules which govern this event.”
Photo consent
The UKWA may authorize or engage an event photographer. By ticking a box and entering any UKWA event I am giving permission for the UKWA to take and publish still and moving images. If you do not agree please complete the photo (non) consent form and send to admin@ukwindsurfing.com The UKWA can not be responsible for other parties who may take photographs during an event.
Medical declaration
Covid
If you answer yes to any of the questions below Today or in the last 14 days you should not attend UKWA events.
- Have you had a high temperature?
- Have you had any of the following symptoms: a new, continuous cough or a loss of, or change to, your sense of smell or taste?
- Have you tested positive for COVID-19?
- Have you been contacted by the NHS test and trace services and asked to self-isolate?
- Are you self-isolating after returning or entering the UK?
- About your household and contacts
- Has anyone in your household or your close contacts had (or does anyone currently have): a high temperature, a new continuous cough or a loss of, or change to, their sense of taste or smell?
- Has anyone in your household or your close contacts tested positive for COVID-19?
COVID Statement
By taking part in any UKWA events, each competitor agrees and acknowledges that they are responsible for complying with any regulations in force from time to time in respect of COVID-19 and for complying with the [venue/OA/sailing club*] COVID-19 procedures. Competitors and support persons shall comply with any reasonable request from an event official. Failure to comply may be misconduct. Reasonable actions by event officials to implement COVID-19 guidance, protocols or legislation, even if they later prove to have been unnecessary, are not improper actions or omissions. A breach of this rule shall not be grounds for protest by another boat. This amends rule 60.1(a).
General

Master Blasters
Master-Blaster entry will be £25, whichever event you choose to enter including insurance for non members. Entry is by the Pay-Pal button on the event page. Please go to the events page and follow the link to the event you wish to enter. By clicking on any PayPal link and entering any UKWA event I am confirming I have read and agree to either The Adult competitor declaration (over 18) or Parents declaration (under 18), and that I have read the Photo consent and Medical declaration.

Refunds
Refunds will only be given in exceptional circumstances, and at the discretion of the executive committee.
